The Use of McKenzie Approach in the Therapy of low Back Pain.

Title of thesis The Use of McKenzie Approach in the Therapy of Low Back Pain. Abstract: The present thesis addresses the issue of treatment of low back pain and in this respect considers the effectiveness of the McKenzie method. The first sections of the theoretical part of this thesis are focused on the kinesiology and pathology of the components of the lumbar spine. Further sections explain the principles of the mechanical diagnosis and therapy, i.e. the McKenzie method. Their focus is to present the specific features and advantages of this approach. The aim of the practical part is to assess the effectiveness of the McKenzie method in patients with chronic low back pain whose previous treatment had no or only a short term effect. Qualitative research based on three case studies has been chosen to achieve this aim. By assessing and comparing both subjective parameters (e.g. the intensity, distribution and frequency of pain) and objective parameters (e.g. the range of motion, neurological symptoms) before and after the treatment by the McKenzie method, its effectiveness for the use in therapy of chronic low back pain was determined. This thesis is focused on the treatment of chronic lumbar back pain using Robin McKenzie method as a therapeutic approach. The theoretical part deals with the physiology of the spine, its pathophysiology and complications related to that. It also presents the McKenzie method, its fundamentals and clinical use. It also describes the Oswestry disability index, which was chosen as a subjective tool for evaluating the effectiveness of therapies from the patient's perspective. The practical part aims to answer the question whether the McKenzie method is a suitable tool for influencing chronic back pain. To answer this question a qualitative research in the form of case studies was made. The key role for assessing the effect of therapy was played by intensity of pain, which was evaluated by the Oswestry questionnaire, dynamics of the spine and neurological symptoms. The results, which were concluded during therapies, argue in favour of the Robin McKenzie method, which is a suitable therapeutic approach to influence chronic lumbar back pain. The thesis is divided into theoretical and practical part. In the first part of the bachelor thesis the author explains the problematics of anatomy and spin physiology, while focusing mainly on the nerve structures, skeleton and muscle system in lumbar area and lower limbs. In the thesis, there is also mentioned the kinesiology of given segments and pathophysiology with the focus on lumbosacral syndroms. Next section of the theoretical part presents detailed elaboration of conservative approaches in the therapy of radiculopaties in lumbosacral area. The aim of the bachelor thesis is to process case studies of two patients who are anamnestically congruent. The author is trying to evaluate, based on given results, which method, either McKenzie therapy or mobilization therapy according to Professor Lewit, led to succesful elimination of painful symptoms, irritation, and especially to modify the postural system into the norm. Discrepancy of extension positions used in physiotherapy

Title: Discrepancy of extention positions used in physiotherapy Objectives: The aim of this thesis is to describe in detail selected extinction position (bhudžangásána, úrdhvamukhašvanásána, McKenzie extension, extension from developmental kinesiology) and then using surface electromyography to verify the similarity and difference of selected muscle involvement in these positions. From these detection then conclude whether this practice often confused position brings the same or different therapeutic effect. Methods: It is the type of quantitative research to theoretical and empirical character. The research method is observing (intra experiment). Using surface electromyography was sensed electrical activity m. trapezius (superior et inferior), m. erector spinae, mm. oblique, m. gluteus maximus and m. semitendinosus in 7 subjects in the implementation of extension positions. Results: It was confirmed that for each of these positions was dominant differnt of the measured muscles. It was confirmed that the position bhudžangásána and úrdhvamukhašvanásána are described in the literature differently than is shown practical experiment.
